Belvedere Hill Ltd – Privacy Statement

On 25th May 2018 a new Data Protection Act comes into effect.

As a controller of data I am required by the new Act to provide the following information about your data: what data is held; how it is stored; how it is used.

I am registered with the Information Commissioners Office (ICO) and should you wish to contact me please do so at the following addresses:

I collect personal information concerning you in order to provide safe and effective psychological therapy. The information includes your name, address, telephone number(s), psychological history, medication etc.

It is vital that I collect your information, as without it, I would be unable to offer therapy or to book and confirm appointment arrangements. You do have the legal right to refuse to supply the information but I would then be unable to provide psychological therapy.

Unless required by your insurance company or other referring agency, your information is held in paper record and note form and held in a securely locked filing cabinet to which only I have access.

As part of my clinical practice ethics (guided by the British Psychological Society’s ethical guidelines), I am required to retain your records for a minimum of seven years after your last clinical appointment with me. Records held after that date will be securely shredded and disposed of.

I will not share your data with anyone (e.g. with your insurance / referring company) without your express consent, unless required to by a court of law.

You have the right to see what personal data I hold of yours following a written request to do so, and also you have the right to ask to correct any factual errors. Provided the minimum period of record retention has elapsed you can also request disposal of your records.

Any breaches of data security that may affect you will be advised to you within 72 hours and the ICO will be informed. I want you to feel absolutely confident that I am handling your personal data responsibly and that I am doing everything I can to make sure that it is secure and cannot be accessed by anyone who does not have your consent to see it.
